The Assisted Reproductive Technology Act 2010 established the Central Register to support
information about donor conceived people, donors of gametes, parents and siblings of children who
are donor conceived and those born through surrogacy arrangements, and to give donors and donor
offspring greater opportunity to access information about each other. The ART Act provides that
the Secretary, NSW Ministry of Health is to establish and maintain the Central Register.
When applying to add, update or access certain information on the NSW Health Central Register,
you will need to provide 100 points of identification before we can process your application.
When you will need to provide identification
You will need to provide 100 points of identification when you make an application:
- To register or update voluntary donor offspring details on the Central Register and consent
for release of information.
- To register or update voluntary donor details on the Central Register and consent for release
of information.
- To register or update non-ART donor offspring details on the Central Register and consent for
release of information.
- To NSW Health (or to an ART provider) for accessible information about a donor.
- As a parent for information about a donor or other offspring of the donor from the Central
Register.
- As a donor seeking information from the Central Register.
Instructions
1.
The 100 Point Identification Check must be lodged with the completed application to the NSW
Ministry of Health Central Register.
2.
Only certified copies of documents should be provided to satisfy the 100 Point Identification
Check. Do not provide original documents.
3.
The identification must be current and include at least one type of photographic identification
and one that contains a signature and date of birth.
4.
The sum of the points for the documents provided must total at least 100 points.
Certification
A certified copy means a true copy of an original document that has been sighted and certified by
an authorised certifier

, commonly either a Justice of the Peace (JP) or legal practitioner. It must
have the certifier's name, title, registration number (where applicable) and be signed, dated, and
note:
'I certify that this is a true and accurate copy of the document/s reported to me to be the
original document/s.’
