California Prenatal Screening (PNS) Program
Instructions For Providers on
Ordering and Collecting cfDNA Screening Blood Specimens
QUEST DIAGNOSTICS LABORATORY
1. Locating a Quest Diagnostics Patient Service Center:
a. Quest Diagnostics has over 500 patient access points throughout Northern and Southern
California.
b. Please use the link supplied below so that your patient can locate the nearest Patient Service
Center (PSC) and schedule an appointment: https://appointment.questdiagnostics.com/as-
home.
2. How to order the cfDNA Screen:
a. Visit the CalGenetic Portal (https://calgenetic.cdph.ca.gov/) to register and order the cfDNA
screen (beginning September 19, 2022).
b. Select Quest Dx as the cfDNA Laboratory. The selection will say Quest Dx (GDSP cfDNA Panel)
CL: 94804005”.
c. After completing the online form, select the SUBMIT button.
d. Print the California PNS Program Consent and cfDNA Order Confirmation on the PNS Program
label stationery provided by the state program*.
i. When the screening order is printed on the stationery, it will contain four PNS Program
barcodes labels.
ii. Obtain the patient’s signature on the PNS Program Consent and cfDNA Order
Confirmation.
3. Specimen collection:
a. In-clinic collection instructions:
i. One barcode label should be affixed to a copy of the patient’s insurance card.
ii. One barcode label should be affixed vertically on each blood collection tube.
b. Collection at a Quest Diagnostics Patient Service Center:
i. Ask the patient to make an appointment at their preferred Quest Diagnostics Patient
Service Center location via the link below: https://appointment.questdiagnostics.com/as-
home.
ii. The patient will be required to bring the following items:
1. A signed PNS Program Consent and cfDNA Order Confirmation form and
2. A copy of their insurance card with the affixed barcode label.

4. Does my patient need to do anything to prepare for the cfDNA screen (e.g., fast)?
No special preparation is needed.
5. How do I request shipping supplies from Quest Diagnostics?
a. The Streck tube and foam cushion (Collection Kit) needed to ensure the appropriate
collection and safe transport of GDSP cfDNA Panel is provided by Quest.
b. The order description is: Kit, NIPT (CELL-FREE DNA) Collection, Product Id: K03
6. Collection Kit ordering options:
a. Online ordering is available using a Quanum account.
i. If you would like to set up a Quanum account, please call 1.866.MYQUEST
(1.866.697.8378) or visit this link: https://portal.Care360.com/
b. Email the Collection Kit order to: warehouseclientsupply@questdiagnostics.com
i. Please include account name and shipping address.
ii. Please use Product ID K03 and indicate the number of NIPT cell-free DNA Collection Kits
needed.
c. Call the Client Supply Department at 1.818.737.6551
d. Fax the Quest Diagnostics Supply Order Form to 1.818.581.4133
i. Please contact your local Quest Diagnostics Sales Representative for the Supply Order
Form.
7. If the blood collection is done outside of the PSC network:
All “GDSP cfDNA Panel” specimens must be kept at room temperature. To ensure timely pickup of
specimens, please schedule courier pickup. To use courier pickup, you must have a Quest account.
a. Please call 1.866.MYQUEST (1.866.697.8378) (Option #1#6).
b. If you already have a Quest account, please schedule your courier pickup using your
Quanum account.
8. What kind of sample is needed (plasma, whole blood, etc.) and what volume should be
collected?
a. The GDSP cfDNA Panel screen from Quest Diagnostics is performed using 10 mL (8mL
minimum) whole blood collected in 1 Streck cell-free (black/tan tiger-top) glass tube.
b. Do not draw before 10 weeks gestation.
c. Sample collection using a straight needle (not a butterfly needle) is preferred.
d. Keep specimen at room temperature, do not refrigerate or freeze.
9. How long after the date of blood collection can specimens be tested?
The GDSP cfDNA Panel screening from Quest Diagnostics is stable at room temperature for 7 days.
